- met name chemo, cocaine, cyclosporine, tacrolimus, ticlopidine, clopidogrel, IViG, anti-D, dapson, nitriet, aniline, hypotonic solutions
- verder, zie onderstaande tabel
- favisme
- lab
- eerder normaal Hb?
- fragmentocyten, ellipto/sfero/ovalo, LGL-cellen (en afwijkingen passend bij loodvergiftiging)
- Coombstest

3.1 TMA
- primary TMA (vaak over dagen)

- HUS (evt. Shiga-toxine gerelateerd)
- DITMA
- complement-mediated TMA (hereditary or acquired)
- rare hereditary disorders of vitamin B12 metabolism (MMACHC gene), or factors involved in hemostasis
- uitlokkend: BLWI, GE-itis, vaccinatie, zwamgerschap (met name post-partum)
- secundaire TMA (–> therapie gericht op onderliggende aandoening)
- bij zwangerschap
- postpartum: complement-gemedieerd
- TTP
- severe preeclampsia/HELLP syndrome
- SLE
- ernstige hypertensie (oorzaak en gevolg, nb tijdsrelatie, TTP zelden nierfalen) > alleen therapie HT
- maligniteit
- infectie: HIV, endocarditis, CMV, parvo
- autoimmune disorders (SLE, aPLS, SSc); bekende ziekte; therapie SLE nuttiger dan PEX
- stem cell or organ transplantation (rejection kidney)
- regimen for bone marrow ablation

5 AIHA
5.1 warmte AIHA
5.1.1 introductie
- warmte-antistoffen
- activiteit bij 37 'C
- vaak IgG, zelden IgM en zeer zelden IgA (dan vaak ook IgG)
- vaak gericht tegen gehele rhesussysteem, zelden tegen het Kell-systeem
- vaak geen C3c/C3d: via Fc-gamma receptor verwijderd via milt
- IgG1 en IgG3 effectieve activator, IgG2 and IgA matig, IgG4 geen
- 50% primair / idiopathisch, 50% secundair
- 18% heeft/krijgt uiteindelijk toch lymfoom
- verhoogd risico op trombose
5.1.2 verplicht aanvullend onderzoek
- nagaan allo / recente transfusie: vertraagde transfusiereactie?
- nagaan inflammatoire darmziekte
- bloedbeeld: onderliggende leukemie?
- lab: standaardlab plus HIV-test, Ig's (CVID?), ANA (10% heeft SLE)
- CT hals t/m abdomen: solide of hematologische maligniteit?
- beenmergonderzoek
- denk aan ALPS
5.1.3 therapie
- altijd
- overleg bloedband voor uitgebreide kruisproeven
- foliumzuursuppletie
- onderliggende aandoening behandelen
- met name medicatie staken
- beruchte medicatie: methyldopa, IFN, AB, NSAIDs, kinine, fluda, lorazepam, oxali, carbo, rifampcine, cimetidine
- ALPS

- 1e lijns therapie
- prednison 1 mg/kg
- bij Hb >6,5 is er sprake van respons, Coombstest mag positief blijven
- bij respons:
- 1 mg/kg nog 1 week volhouden
- daarna in 2 weken afbouwen naar 20 mg/dag, dat 1 maand volhouden
- meestal: ieder 2 of 3 dagen 10 mg minder
- dan per 2 weken 2,5 mg afbouwen

- CA-Ab frequently directed to I-antigen or H-antigen
