1 DIRA
daratumumab = IgG kappa monoclonal antibody little M-spike in the immune electrophoresis and immunofixation low amount of IgG kappa (approximately 1 g/l) DIRA: “Daratumumab Interference Reflex Assay”
- anti-daratumumab antibodies: daratumumab IgG kappa is removed
- if after the DIRA-test an IgG kappa is still present, it must be the original M-protein
- this means no CR, retest the patient until DIRA-test is negative.
- with non-IgG kappa m-protein not necessary
- with no VGPR not necessary

- use of daratumumab: risk of Hepatitis B (HBV) reactivation
