daratumumab = IgG kappa monoclonal antibody little M-spike in the immune electrophoresis and immunofixation low amount of IgG kappa (approximately 1 g/l) DIRA: “Daratumumab Interference Reflex Assay”

anti-daratumumab antibodies: daratumumab IgG kappa is removed
if after the DIRA-test an IgG kappa is still present, it must be the original M-protein
this means no CR, retest the patient until DIRA-test is negative.
with non-IgG kappa m-protein not necessary
with no VGPR not necessary
