Polyneuropathie en M-proteine

1 Verplichte diagnostiek
- kliniek: bij M-proteine vaak distaal sensorimotorisch
- soms puur sensorisch (met name bij anti-MAG, maligniteit)
- bij amyloidose vaak ook autonome component
- EMG
- axonaal: amyloidose, cryoglobulinemie, Castleman, CIAP
- demyeliniserend: IgM-MGUS, POEMS, CIDP en vasculitis
- geen bewijs dat IgA/IgG-MGUS en M-proteine iets anders is dan coincidentie
- of dat de toxische therapie nuttig is cq. beter dan de therapie voor CIDP
- indien IgM: anti-MAG-antistoffen
- uitsluiten onderliggend lymfoom dan wel multipel myeloom
2 Beleid
- bij toename polyneuropathie progressie (tot) maligniteit uitsluiten
- bij onderliggende maligniteit behandeling maligniteit te overwegen
- demyeliniserend
- overweeg therapie voor standaard CIDP (IVIg, steroiden, plasmaferese), vaak effectiever dan anti-MGUS-therapie, met name bij IgA/IgG1
- bij IgM-MGUS met ernstige/invaliderende en progressieve polyneuropathie therapie te overwegen, zeker bij anti-MAG-antistoffen: monotherapie rituximab of DRC (aangezien CR waarschijnlijk nodig is)
- axonaal
- zoek naar onderliggende alternatieve verklaring
- vermijdt contact met toxine: glucose, medicatie, alcohol, systeemziekte, hypothyreoidie
