- bij geen verklaring: autoimmune workup
- alleen arteriele trombofilie
2 trombofilie
- alleen veneus
- erfelijk
- Factor V Leiden
- Prothrombin Mutation
- Protein C/S Deficiency
- erfelijk
- veneus en arterieel (dan vaker veneus dan arterieel)
- HIT
- hyperviscositeit: sikkcelcellen, M-proteine, leukocytose
- MPN: afwijkend bloedbeeld of splenomegalie
- PNH: hemolyse, pancytopenie
- TTP/HUS
- DIS (met name indien chronisch, acuut vaker bloeding)
- dysfibrogenemie (meestal bij leverziekte)
- cryofibrogenemie
- cold-insoluble proteins composed of fibrinogen, fibrin, and fibronectin, that leads to cold sensitivity, Raynaud’s purpura, urticaria, skin ulcerations, and arterial and venous thrombosis
- wordt gezien bij autoimmune diseases, malignancy, and infection
- cryofibrogenemie
- hyperhomocysteinemie?

- therapie middels foliumzuur negatief

4 APS
- antifosfolipiden syndroom: zwangerschapscomplicaties, trombopenie
- primair / idiopathisch
- secundair
- Autoimmune Disorders (especially SLE)
- Malignancy
- Infections (bacterial, virla, and parasites)
- Drug Reactions
- Diagnostic Criteria:
- Clinical:
- 1 or more episodes of venous or arterial thrombosis, and/or
- pregnancy complications
- unexplained fetal loss after 10 weeks, or
- 3 or more unexplained fetal losses prior to 10 weeks, or
- 1 or more premature birth prior to 34 weeks
- klassiek:
- young stroke zonder risicofactoren
- vaker veneus maar volgt vaak patroon van debuut
- vaak vreemde lokaties
- trombopenie / ITP
- TMA
- huid: splinter hemorrhages, livedo reticularis, skin necrosis/infarction and digital gangrene
- valvular thickening and vegetations that can be infectious or non-infectious
- Clinical:
- Laboratory: APL Abs on 2x, >12 weeks apart
- Lupus Anticoagulant
- ~50% prolonged aPTT met positieve mengproef
- Dilute Russell Viper Venom Time (dRVVT) more sensitive
- Anticardiolipin Antibodies
- IgG is more specific than IgM
- Usually moderate-high titer
- Lupus Anticoagulant
- Transient ACL Abs positivity
- ~10% healthy patients, usually low-titer
- persist in only ~1-2%
4.1 therapie
- geen primaire preventie tenzij SLE: ascal / plaquenil
- DVT: als gewoonlijk, hogere recidiefkans
4.2 catastrofaal APS
- <1% patienten met APS
- criteria, problemen binnen 1 week van elkaar
- uitgebreide trombose
- multiorgaanfalen: 3
- laboratoriumaanwijzingen APS
- PA: kleine vaten-occlusie
- therapie
- i.v. heparine
- hoge dosis steroiden
- plasmaferese bij TMA
hoofd-halstumoren en histologisch naaldbiopt
- bij lage Gauge-waarde veilig
