1 Diagnostiek
- oculocutaan albinisme and at electron microscopy absence of dense bodies
- genetic testing.
1.1 DD
- Chediak-Higashi syndrome: the number of platelet dense bodies is also severely reduced, but giant intracytoplasmic granules are also seen; recurrent pyogenic infections, progressive neurologic abnormalities, mild coagulation defects, and a lymphoma-like accelerated phase
- oculocutaneous albinism: mild phenotype presenting with unrecognized bleeding tendency; conversely, oculocutaneous albinism type 1 may display easy bruising
- Griscelli syndrome: autosomal recessive, pigmentary dilution of skin and hair, presence of large clumps of pigment in hair shafts (pathognomonic), and variable association with immunodeficiency, neurologic abnormalities, and manifestations of hemophagocytic lymphohistiocytosis
2 Therapie
- huid
- levenslange zonbescherming
- zon vermijden tijdens piekuren van de dag
- dragen van hoed met wijde rand en zonnebril
- dagelijks gebruik van breedspectrum zonnecreme van minimaal factor 30
- zon vermijden tijdens piekuren van de dag
- huidonderzoek a 6-12 maanden vanaf adolescentie en uitleg over zelfonderzoek
- levenslange zonbescherming
- behandeling van visusproblemen met bril of contactlensen
- bloedingsneiging
- uitleg
- dragen van kaartje of ketting met medische waarschuwing en diagnose
- oppassen met vaccinaties (cave intramusculaire bloeding)
- vermijden van NSAIDs
- evt. couperen van menstruaties
- evt. zachte tandenborstel
- beschermende maatregelen voor chirurgische/tandheelkundige ingrepen, bijv. DDAVP
- bij kleine bloeding druk met gaasje met vaselline (en evt. met trombine daarin opgelost)
- dragen van kaartje of ketting met medische waarschuwing en diagnose
- DDAVP-test
- bij ernstige bloeding of chirurgie: DDAVP of plaatjestransfusie
- terughoudend met plaatjestransfusies vanwege risico op alloimmunisatie
- uitleg
- bij type 1 of 4 en buikklachten colonoscopie, en evt. systemische corticosteroiden of andere behandelingen zoals bij ziekte van Crohn
- longfibrose (bij typen 1, 2, of 4)

- jaarlijkse griepprik/pneumococcenvaccinatie
- aggressieve behandeling infecties

- pirfenidone nog niet in NL geregistreerd
- jaarlijks longfunctie en bij afwijkingen HR-CT (om stralingsbelasting te beperken met tussenpozen van 3-5 jaar tenzij klachten toenemen of longfunctie verslechtert)
- longtransplantatie, te bespreken zodra fibrose toeneemt en waarbij bloedingsproblemen geen contraindicatie zijn

- infecties: G-CSF bij neutropenie, antibiotische profylaxe
- verwijzing naar geneticus voor bereiken van vruchtbare leeftijd
3 Kliniek
- autosomal recessive, high incidence northwestern region of Puerto Rico
- oculocutaneous albinism
- ook haar aangedaan
- prominent signs of solar skin damage
- nystagmus, sensitivity to bright light, iris transillumination, reduced visual acuity
- ook haar aangedaan
- bleeding diathesis (platelet storage pool disorder)
- other organ involvement specific to the subtype
- granulomatous colitis resembling Crohn clinically and histologically
- HPS types 1 (30%) and 4
- begin during adolescence, colon in most cases, metastatic skin lesions have been reported
- HPS types 1 (30%) and 4
- pulmonary fibrosis
- similar to idiopathic pulmonary fibrosis, irreversible and progressive
- in HPS types 1 (80%), 2, 4; in the early 30s in HPS type 1, earlier in HPS type 2
- slow onset of dyspnea on exertion or at rest and a nonproductive cough, ultimately leading to the need for chronic supplementary oxygen and respiratory failure
- leading cause of death, life expectancy approximately 10 years after diagnosis of restrictive lung disease in the absence of lung transplantation
- similar to idiopathic pulmonary fibrosis, irreversible and progressive
- recurrent infections due to severe neutropenia and immunodeficiency have been described
- types 2 and 10

- cardiomyopathy and renal failure have been rarely reported
- granulomatous colitis resembling Crohn clinically and histologically
4 Pathofysiologie
- homozygous or compound heterozygous mutations in 1 of 11 genes
- that support intracellular biogenesis and trafficking of lysosome and lysosome-related organelles (melanosomes, platelet dense bodies, lamellar bodies of type II pneumocytes, granule proteins of cytotoxic and suppressor T cells and natural killer)
- hypopigmentation: impaired melanosome formation, trafficking, or transfer to keratinocytes, while the melanin biosynthesis in melanocytes is unaffected
- platelet dense bodies are absent
- organ damage: unclear
